SMAS Facelift from Dr. Tansavatdi

With age comes the loss of facial elasticity, along with laxity in the superficial muscular aponeurotic system (SMAS), which is a layer of tissue deep within the skin that also happens to be an incredibly important support structure for the face. The loss of elasticity and the weakness in the SMAS membrane causes the cheeks to sag along the jawbone, which results in fleshy jowls, and typically a double chin. Dr. Tansavatdi’s SMAS facelift combats these issues by tightening up the muscle and decreasing fat and excess skin.

What is a SMAS facelift?

A SMAS facelift targets the lower third of the face, and it is usually recommended when cheek fat sags forward and emphasizes the marionette lines due to the weakness of the SMAS membrane. A SMAS facelift is less invasive and requires less recovery time than a traditional facelift. It can address the following issues:

  • Sagging of the mid-face
  • Jowls
  • Sagging excess skin/fat

Candidates for a SMAS facelift

The best candidates for a SMAS facelift are in their 40s and early 50s and have some facial sagging, but also still have some elasticity in the skin. Candidates must also be relatively healthy non-smokers. 

What to expect during surgery

A SMAS facelift may take a few hours, depending on the patient’s specific needs. When you arrive for your SMAS facelift procedure, you will be prepped for surgery. Dr. Tansavatdi prefers performing these procedures under general anesthesia for the comfort of her patients at an accredited surgical facility. For the right candidates, she can also perform this procedure under local anesthesia.

Dr. Tansavatdi will make an incision at the temple, just above the hairline. This incision will extend downward and will be hidden along the natural creases of the skin, curving behind the ears. Once this is complete, the tissue and muscle beneath the skin will be lifted and moved into a higher position. Excess skin will be removed, and the skin will be placed back over the face. The incisions will be closed with sutures, and most of the scars will be hidden within the hair and natural creases of the skin.

Downtime and post-care

After your SMAS facelift, you will have a wrapping around your head that will need to be removed the following day. Dr. Tansavatdi is meticulous about controlling all bleeding during surgery, and applies a clotting spray in order to avoid using drainage tubes (which can be cumbersome for patients). Expect to have some numbing, swelling, bruising, and discomfort for about two weeks. This also marks the time when you should be able to return to work, continue exercise, and resume most daily activities. Dr. Tansavatdi recommends avoiding activities that involve excessive turning of your neck for an additional two weeks.
